Grind, Sell Early, or Redesign? Three Roads When Your Practice Runs on You

When your practice runs through you, three roads open up: keep grinding, sell early, or redesign your role first. How to choose one on purpose instead of drifting into it.

You built something good. Your hands, your judgment, your name.

Now every road forward runs through you, and you are choosing between them in the gaps between patients.

Here is the short answer. When your practice runs on you, you have three roads. Keep grinding. Sell early. Or redesign your role first. Any of them can be the right one. The risk is not picking the wrong road. The risk is drifting onto one without deciding.

The strength that narrows the choice

You became an owner because you were very good at the work.

Patients asked for you. Your team learned to bring you the hard calls. That is how a strong practice gets built.

It is also how your options shrink. The more your practice leans on your skill, the fewer roads feel open. So it helps to name them plainly.

Road one: keep grinding

You keep the model you have. You work harder inside it.

This road is familiar. It pays. It asks nothing new of you on Monday morning.

It also keeps every decision on your desk. Your schedule fills before you see it. The bigger questions wait for a quieter month that never comes.

Grinding is a fine road for a season. It is a hard road for a decade. And the longer you stay on it, the more the other two roads get decided for you.

Ask yourself: Is this a season, or is this my plan?

Road two: sell early

You take an offer now. Maybe a DSO. Maybe an associate. Maybe a partner group.

This road can be exactly right. Your health, your family, your energy and the market all count. Selling soon is not a failure. For some owners it is the clearest choice.

The trouble is how the choice gets made. A letter arrives. It sits on your desk. You read it between crown preps. A deadline shows up, and the deadline decides.

If you sell, sell on purpose. Know what you are trading and why.

Ask yourself: Am I choosing this, or is the timeline choosing for me?

Road three: redesign your role

This road starts with a different question.

The common question is "When do I sell, and to whom?" There is an earlier one. What do you want your role to be?

For some dentists, the next chapter is not retirement. It is a different job inside the same building, or outside it.

You might want to:

  • Stay clinical and let someone else run the business.
  • Teach and mentor the next dentist.
  • Own without operating.
  • Step out fully, on a date you set.

Each of those is a role. Each one asks your practice to work a little differently. Redesign means you decide which role you want, then shape your practice and your plan around it.

Redesign does not promise a better price. A broker or qualified appraiser sets what your practice is worth. Redesign is about choosing your next years, not raising a number.

Ask yourself: If no one were waiting on my answer, what role would I pick?

The decision needs its own time

Here is the part that gets skipped.

One of the biggest decisions of your career should not be made between patients. It needs a protected block on your calendar.

Not a retreat. Not a weekend away. One recurring block, outside the operatory, that belongs to this decision.

Give that block one written question. Here is a good one:

If you had nothing left to prove, what would you choose?

Read it at the start of each block. Write down what you notice. Over a few weeks, the answer gets clearer than it ever does in a hallway.

One move this month

You do not need a full plan to start. You need one move.

Pick one thing you will do this month. Give it a date. Tell one person who will ask you about it.

It can be small.

  • Block the time and keep it.
  • Write down which road you are on today, and whether you chose it.
  • List the roles you would enjoy, and the ones you would not.
  • Ask your spouse which road they think you are on.

Then finish one sentence. "This month I will ___, so that ___."

One move with a date beats a perfect plan with none.

You are the one deciding. Make it a decision.
